If you were in an accident and would like to use your Personal Injury Protection at our office, please call to make an appointment with the front desk. On the day of your appointment please arrive 15-20min early and bring the following documentation and information with you for your initial visit:

1. Auto Insurance Card
2. Auto Claim Number
3. Address to mail claims - sometimes located on the back of your auto insurance card
4. Drivers License
5. Adjusters Name
6. Social Security Number
7. Any and all Images/Imaging reports you may have
